The easiest way is by setting up an applicant program. All these questions can be set up as additional info for the student. You would need to call in to tech support to create custom reports for the info you collect.

There is a fast entry screen under additional info.

what do you mean by "applicant program"?

You enter in admin under programs a new program called applicant.  You enter the child in the system and istead of entering a primary period you only enter the period and class in the classes tab.
