Meningitis second shot rules

I have found that i needed to add the following rules to the second Meningitis shot for my 12th graders. As many were getting the second shot before their 16th birthday thus requiring them to get a 3rd shot.  (https://www.health.ny.gov/publications/2370.pdf read note 8. c.)

We have now updated our default rule. So if you have 12th Grade update the software and select the row of menigitis and click on default value on the bottom right corner, and enter no on the first message box. This will reset the rules to the new default value.
